Professor Patrick Hudson

Professor Emeritus Patrick Hudson is one of the world’s leading authorities on the human factor in the management of safety, in the oil and gas industry, commercial aviation and medicine. He was the Project Leader of the Tripod Research program for Shell International and was involved in developing Shell’s approach to safety management systems after the Piper Alpha disaster.

His involvement with aviation includes working with companies such as British Airways, Cathay Pacific, Malaysian and Singapore Airlines, and Airbus. In medicine he has concentrated upon bringing his experience in industrial risk management, especially in the area of medication error. He also advises other industries, including the mining industry and was also appointed to the safety oversight board of the European Union’s main nuclear research reactor. More recently he is a testifying expert on process safety and safety culture in the trial in the US 5th Federal court in New Orleans on the BP Deepwater Horizon disaster in the Gulf of Mexico.

Brent Hayward

Brent Hayward is the Managing Director of Dédale Asia Pacific. He is a professionally qualified and Registered Psychologist, with more than 35 years’ experience in the provision of services and advice to management within the Aviation industry. Previous employers include the RAAF Psychology Service, Australian Airlines and Qantas Airways.

Brent has developed and delivered specialist training in aviation psychology, human factors, CRM, and aviation safety Investigation methods for a range of organisations in Australia, Africa, Asia, Europe, the Middle East, the Pacific, & North and South America. This has included human factors and safety investigation training seminars and workshops conducted on behalf of the European Association for Aviation Psychology, EUROCONTROL and for the Singapore Aviation Academy.

He has had considerable experience in the Investigation of human factors aspects of both military and civil aircraft accidents and incidents. Brent also has applied human factors and safety management experience within a variety of other industries such as rail, energy and healthcare.

Dr Graham Edkins

Dr Graham Edkins is a qualified Organisational Psychologist and internationally regarded human factors and safety management systems expert with broad experience as a transport safety regulator, group safety manager and independent investigator. As a former Air Safety Investigator (Human Performance) with the Australian Transport Safety Bureau (ATSB), Manager Flight Safety Investigations for Qantas Airways, Executive Director of Public Transport Safety Victoria and Group General Manager of the Civil Aviation Safety Authority (CASA), he brings a wealth of experience and detailed knowledge of contemporary safety regulation, major event investigation and human error management practices.

Dr Edkins has represented Governments on a number of national forums, including the Standing Committee on Transport (SCOT) Rail Group, Safety Advisory Program for Commercial Aviation Operators (SAPCOM), Australian Representative and Vice Chairman, International Air Transport Association (IATA), Human Factors Working Group and the National Steering Committee for the development of National Rail Model Safety Legislation. He is regularly retained as an independent safety investigator and human factors expert witness for various civil and criminal matters. Additionally, he provides ongoing safety and human error management coaching services to several company boards and executives in a range of safety critical industries.

Captain Todd Mickleson

Captain Todd Mickleson is a current B737 Captain and the Human Intervention Motivation Study (HIMS) representative and pilot peer support program (PSP) representative for a major international airline in the Asia Pacific region.

He is a qualified Flight Instructor, type-rated on the B737, B747, and B757/767 aircraft, and has accumulated more than 15,000 flying hours, including 8,500 hours as Pilot in Command. Captain Mickleson has a Bachelors’ Degree in Aviation, majoring in Human Factors and Safety Management, and has been involved in the development and implementation of Safety Management Systems (SMS) and Human Factors (HF) programs at all levels of the aviation industry for more than 20 years.
